Falls Seminar - 21st February 2007

"PREVENTING BOUNCE BACK" -
TOWARDS INTERGRATED FALLS PREVENTION AND MANAGEMENT

Background

Do you want to see the numbers of people attending ED as a result of a fall decrease? Read the evidence and examples of strategies that are effective in preventing the bounce back. Workshop with those in your region to see what can be done to increase referrals from ED to GP’s, Allied Health etc to manage and decrease Falls Risk.

The falls seminar was coordinated by Active Ageing Australia®, with funding from the South Australian Department of Health – Health Promotion Branch.

Michele has a background in health promotion research and policy at state and national levels.

Andrea Lomman,
Senior Policy Officer – Clinical Network Development Unit
Clinical Policy and Programs, Department of Health WA
She provides support to policy areas relating to the Clinical Networks and Falls Prevention. She has a Bachelor of Science, Occupational Therapy and a Post Graduate Diploma in Rehabilitation and has worked both as a therapist in health services as well as a senior project officer.

Dr Chris Bollen,
Executive Director, Adelaide North East Division of General Practice, Modbury
Chris graduated from the University of Adelaide with MBBS in 1987 and has been a General Practitioner at Ingle Farm, north east of Adelaide since 1990. He is also an AGPAL surveyor (accrediting general practices), Executive Director at Adelaide North East Division of General Practice, and Coordinator of GP Training at The Queen Elizabeth Hospital.

Rosalind Wren,
Occupational Therapist, Resthaven Community Services, Elizabeth
Rosalind worked in the acute sector in geriatric and psycho-geriatric medicine and rehabilitation, before moving to Adelaide. For the last 10 years Rosalind has been working in Therapy Services for older adults in community settings, and has 20 years experience in aged care.

Deb Charlish,
Emergency Department Discharge Coordinator and;
Debbie McCarthy,
Senior Social Worker, Emergency Department, Flinders Medical Centre, Bedford Park
The Coordinator’s role was introduced in July 2006 to facilitate good discharge planning, bridging the gap between the acute care sector and the community to prevent bounce back. Deb has 32 years of extensive nursing experience, and a Post Graduate Certificate in Critical Care and has spent the last 4 years working with Hospital @ Home at Flinders Medical Centre.

Debbie coordinates a seven day service providing psycho social assessments to address crisis situations, hospital avoidance and trauma counselling for the patients in Emergency. Deb also provides education and debriefing for staff re critical incidence.
